Instructions for Patients:
Step #1 -- Fill in and send the "Request for Appointment Form" located by the picture of the Health Provider of your choice.
Step #2 -- The Health Provider will respond to your request via e-mail and book a V-Telehealth consultation appointment with you.
Step #3 -- Once the appointment has been scheduled simply click on the Health Provider's picture and a "Room Signon" screen will appear.

Step #4 -- Simply type in your name and click on the link to enter the Health Provider's virtual office where the two of you can have a completely private and secure video conference.
Please note that the Health Provider may also have an associate attending the session for the purpose of taking notes which can be recorded into your confidential health file maintained by your Health Provider.
